The following information comes from the Clinton County Department of Health and is compiled from the inspection report.

The violation is serious or non-serious. Serious violations are more likely to cause food contamination, disease or environmental health hazards, including improper food cooking or poor personal hygiene.

• Duchess Shoppe, 2495 Rombach Ave., Wilmington, November 10.

The key: no verifiable employee knows when to report an illness. There are knives, pizza cutters and towels in the sink. I moved them so that I could wash my hands. All sinks are for hand washing only. In the take-out refrigerator, there are two types of blueberry parfaits whose use date is November 9. The use date of the chocolate pudding, blueberry parfait, and banana pudding containers in the reach refrigerator is November 9. A display of biscuits within reach, the chocolate/peanut butter biscuits will be used on November 9th. Half and half of the creamer dispensers are not out of date. In the walk-in cooler, the roast beef is not obsolete, and the salami in the Ziploc bag is not obsolete.

Non-critical: The staff who handles/makes food have ponytails, but there are no hair restrictions. The inside of the kitchen oven is dirty. The floor tiles in front are broken, go cool down. Two broken doors under the coffee pot (lost). Box on the floor of the walk-in cooler. Garbage on the floor of the walk-in cooler.

• Burger King, 1607 Rombach Ave., Wilmington, November 9th.

Key: The inner edge of the ice machine and the inside of the door are dirty. The inside of the ice dispenser of the soda machine is dirty. French toast sticks, potato pancakes and sausage patties are not frozen in a deep fryer that can be put in the refrigerator. The door is not closed; there is water at the bottom of the device, and the temperature of the device is 65°F. All products are removed and processed by the manager. Thank you. The freezer must be maintained/repaired before being used again.

Non-critical: Tuck paper towels under the microwave/place on the counter to absorb moisture. The outside of the walk-in freezer at the back was dirty with black mildew. The two microwaves in the kitchen are dirty. The counter behind the steamer/toaster is dirty. The interior of the prepared sink, hand sink, and 3-compartment sink is dirty. All sinks must be properly cleaned to prevent any possible cross-contamination. The floor under the soda machine in the lobby is leaking. The floor behind the ice machine is dirty. The floor of the walk-in cooler around the door is dirty.

• Denver Elementary School, 291 Lorish Ave., Wilmington, November 15.

Non-critical: You can find raw soda in the clean dish area. The drinks have been relocated. correct. The kitchen exit door is not fully locked/closed (stretched open). The external openings of the facility must be sealed to prevent the entry of flies/pests. correct.

• Little Caesar's, 1334 Rombach Ave., Wilmington, November 9. Follow-up.

Non-critical: No one here is a food protection certification manager. All facilities must have at least one certified food manager.

• Snow Mountain Country Club, 11093 SR 73, New Vienna, November 8.

Severe: Hot water is not available. The person in charge stated that the water is heated on the stove for cleaning. The water heater is under repair. The peppers are dated in the pre-cooler on November 1st. The person in charge discards and corrects.

Non-critical: lack of thermometers in insulation equipment. Fly bars (control devices) near the 3 sinks. Mouse droppings were observed in the corner behind the ice machine; monitor to determine past or current pest activity.

• Eagles Lodge, 141 E. Sugartree St., Wilmington, November 9.

Non-critical: No one here is certified as a food protection manager.

• Clinton-Massie Middle School, 2556 Lebanon Road, Clarksville, November 9.

Non-critical: Dishwasher meters are not measuring the correct temperature (poor maintenance). The light bulb does not work in the refrigerator.

• Clinton-Massie High School, 2556 Lebanon Road, Clarksville, November 9th.

Not critical: latex gloves were found in the washroom. The person in charge is removed from the facility.

• Countryside YMCA CM Prime Time, November 9th, 2556 Lebanon Road, Clarksville.

Non-critical: a napkin bag stored on the floor.

• Clinton-Massie Elementary, 2380 Lebanon Road, Clarksville, November 9.

Non-critical: It does not work slightly above the steam well platform. The person in charge stated that a replacement bulb has been ordered. Ice accumulates in the walk-in refrigerator. The person in charge stated that a repair request has been submitted.
